过程现象：
X年10月30日18时04分3A凝结水泵滤网堵，3B凝结水泵两次备用状态突然启动，十几分钟后推力轴承温度持续升高至91℃(第二次轴承温度很快升至75℃停泵)，轴承冒烟。
原因分析：
1) 油中带水，因冷却水管垫直径较大，密封不严水进入轴承箱，同时更换冷却水管时不注意水从轴承压盖与轴的密封处进入轴承室，运行初期泵旋转中，油质不良但油与水并未分层，故障冷却效果虽差，未造成烧瓦，当泵停止备用时，油水分层，积聚在底部，推力瓦大部分浸在水位形不成油膜，在启动初期即很快烧毁，(温度显示并不高70)且造成油质进一步恶化，引起导瓦烧毁。
2) 检修过程中换油时，电建放油未从底部放油丝堵放油，而是从推力轴承温度变送器处放油，积水无法排出，油乳化造成推力瓦烧毁。
3) 凝泵推力轴承存在问题。
4) 泵的安装工艺存在问题。
5) 油位过高，油温较高；45-55mm即油标的1/4～1/3；
6) 检查冷却水量设计偏低;
7) 闭式冷却水进水与旁路调整门同时关闭造成冷却水断水。
防范措施：
1) 加强油质检查。
2) 倒泵时应注意观察新启动泵的运行状况良好，方可停止运行泵。
3) 切停泵时必须解除联锁。
